Becoming an adult

Things start to change as you become a young adult.

People will work with you and your family, to help make a plan for your future.  This includes your school.

Your plan will help you to be as independent as possible, when you are an adult, so you have a good life.

Your plan will help you think about your future after school.  It will include things that are important to you like:

• Where you live
• Having fun

• Your family
• Your friends

• School and college
• Going to work

• Staying safe
• Staying healthy

Your plan will be reviewed with you every year or more often if you need to.  This is to make sure things are going well.

Remember, you are the most important person in your plan.  It is okay to ask questions and make sure that people know what you want.
