Herefordshire Register of Children with Disabilities
Herefordshire Council has a duty to hold a register of disabled children and young people within the county, in accordance with The Children Act 1989.
To register your child, they must be under 18 years old and have a physical or learning disability which is likely to have a substantial and long-term adverse effect on their ability to carry out usual day to day activities.
The register includes the following information:
- Name of child or young person
- Address
- Date of birth
- Ethnicity
- Names of parents / guardians / carers
- Contact details
- Details relating to the child's disability and services accessed
The council will use this information to:
- Ask children, parents and carers for their help when planning the development of services for disabled children and young people. For example, it helps them develop appropriate services to meet an individual's assessed needs, in the places where they are most needed
- Provide statistical information for planning, monitoring and evaluating services
- Keep children, parents and carers informed of new or amended services, as they become available
Please note: Registration is voluntary and is not linked to, or a requirement of, receiving social care services which are assessed separately.
Privacy of information
From time to time, general information on the number of disabled children and young people, and the area where they live, may be published. However, no information that could be used to identify you or your child will ever be made public and information held on the register will be treated as strictly confidential.
The information you provide will be kept until your child turns 18, after which it will be deleted. You have the right to access information the council holds on you or any child you have parental responsibility for, subject to the Data Protection Act 2018, including the right to request access to your records, rectify inaccurate data and withdraw your consent.
You also have the right to make a complaint about how Herefordshire Council handles your personal data via the council’s Data Protection Officer on 01432 260616 or informationgovernance@herefordshire.gov.uk or to the Information Commissioner’s Office.
As is the case with any information held by Herefordshire Council, it may be disclosed without consent to safeguard a child, children or adult, who may be at risk of suffering significant harm or to comply with any legal obligation, to which the council is subject.
In order to comply with the Data Protection Act 2018, the council needs informed consent for all existing and new entries on the register. Therefore, if you were previously registered, you will need to re-register to be included on the Herefordshire register of children with disabilities.
To register or re-register your child, you can either complete the online registration form or download a registration form to be returned via e-mail or post using the details on the form.
If at any point in the future, you no longer wish your child to be included on the register or if you have any questions please contact Herefordshire Council's Additional Needs Business Support Team on 01432 261584 or DisabilityRegister@herefordshire.gov.uk.
