Have you used the Community Equipment Service?

Herefordshire Council, Wye Valley NHS Trust and Herefordshire Clinical Commissioning Group invite residents who’ve used the county’s Community Equipment Service over the last five years, to tell them about their experiences.

The service is currently run by NRS Healthcare Ltd, which loans residents a wide range of medical equipment, such as hospital beds, hoists, commodes, riser recliner chairs and telecare equipment.

The information you provide will be used to further improve the service and there are several ways to submit your feedback:

Online: www.herefordshire.gov.uk/communityequipment

Written: Collect a paper survey from any local library or request a copy by calling 07792 002050 or e-mailing
communityequipment@herefordshire.gov.uk

In person: Drop into Hereford Library on Tuesday 9 April between 10am and 12pm

The deadline for all completed surveys is Monday 22 April 2019.
